Additional Paperwork

The following paperform forms may also be needed. Click on a form to download it:

1. Doing Business As (DBA)

This form must be filled our customer check has a different name -or- Doing Business As a different name than what is noted on the account paperwork. Example: "Smith Farms" when an account is "William Smith". The DBA name must not be a separate entity.

2. Additional Account Request Form

This form can be completed in order to "add on" or open an additional account when a customer has an existing open futures account with FuturesOne. The purpose would be to trade a separate trading or hedging program, but avoid filling out new paperwork when the account ownership and information is the same.

3. Individual Proprietary Letter

This form certifies that the funds deposited in the specified account are the individual's and do not represent the interests of any other pools, individuals or entity.

4. Corporate Proprietary Letter

This letter needs to completed and signed by the individual officer acting as the operating manager of the Corporate account.

5. Sole Ownership Corporate Proprietary Letter

This letter needs to be completed if an individual is acting as President and Sole Shareholder of the Corporation in which the account is opened as.

6. Sole Proprietorship Certification

States that the client is doing business as a sole proprietorship and is the sole owner of the proprietorship.

7. Supplemental Customer Information

This is a non-solicitation letter that Man requires all foreign accounts to be complete.

8. Letter of Direction & Additional Disclosure Information For New Accounts

Allows the customer the ability to give direction to follow a specific CTA recommendation or program.

9. Trust Amendment

Amendment for living tusts to be able to trade futures and forex accounts.

• Foreign government, international organization, foreign central bank of issue, tax exempt organization or private foundation - W-8EXP or W-8ECI
• Person acting as an intermediary - W-8ECI or W-8IMY
• Person claiming an exemption from US withholding on income effectively connected with the conduct of a trade or business in the United States - W-8ECI
